1:1 Service Agreement & Liability Waiver

I understand that services provided by Lore Goldstein and Soul Wisdom Enterprises LLC are educational and wellness-based in nature and are not a substitute for medical care, physical therapy, chiropractic care, diagnosis, or treatment from a licensed healthcare provider.

I understand that Lore Goldstein is not a physician, physical therapist, or orthopedic specialist, and does not diagnose medical conditions, prescribe treatment, or provide medical advice. Any information or movement instruction provided is intended to support general wellness, muscular function, movement education, strength, stability, and body awareness.

I understand that muscle activation, yoga, corrective movement, breathwork, and related wellness practices may help improve movement patterns, muscular support, and overall physical function, but do not correct structural abnormalities, fractures, tears, degeneration, or other underlying medical or orthopedic conditions.

I acknowledge that it is my responsibility to consult with an appropriate licensed healthcare professional regarding any pain, injury, medical condition, surgical recovery, or physical limitation prior to participation. I understand that persistent, severe, worsening, or unexplained symptoms should be evaluated by a qualified medical provider.

I certify that I am voluntarily participating in these services and understand that all physical movement carries inherent risks, including but not limited to muscle soreness, strain, aggravation of existing conditions, falls, injury, or other physical discomfort. I knowingly and voluntarily assume all risks associated with participation.

I agree to immediately communicate any pain, discomfort, dizziness, limitation, or concern during sessions and understand that I may stop participation at any time.

I understand that no guarantees or warranties have been made regarding outcomes or results.

To the fullest extent permitted by law, I release and hold harmless Lore Goldstein and Soul Wisdom Enterprises LLC from any and all liability, claims, demands, damages, costs, or causes of action arising from participation in services, except in cases of gross negligence or willful misconduct.

By booking and participating in services, I acknowledge that I have read, understood, and voluntarily agree to this waiver and release.
